TREATMENT STRATEGY OF REDO AORTIC VALVE REPLACEMENT AFTER THE APPROVAL OF TRANSCATHETER AORTIC VALVE-IN-SURGICAL AORTIC VALVE

Transcatheter aortic valve-in-surgical aortic valve (TAV-in-SAV) is now covered by insurance for reoperation after aortic valve replacement due to valve degradation. The early results of TAV-in-SAV are better than redo surgical aortic valve replacement (SAVR) due to its minimally invasive nature. However, the results of TAV-in-SAV for small surgical valves remain dependent on pressure gradient, and the current indications for TAV-in-SAV are limited. To implement such a strategy, it is imperative that surgeons have the skills and knowledge to perform both techniques (redo SAVR and TAV-in-SAV). We sincerely hope that surgeons will perform minimally invasive surgery for structural heart disease.
