THE RESEARCH OF PREOPERATIVE COMBINED THERAPY OF ESOPHAGEAL CANCER

Since 1958 in second surgical department of Chiba university hospital, as preoperative combined therapy of esophageal cancer, through the experience of irradiation alone and irradiation + BLM combined therapy, irradiation + BLM + immunotherapy combined therapy was originally carried out after 1975. The authcr divided this preoperative combined therapy into 5 groups, as follows, group I; without preoperative combined therapy, group II; irradiation alone, group III; BLM alone, group IV; irradiation + BLM, group V; irradiation + BLM + immunotherapy (PS-K and BCGCWS). From 1968 to 1977, 167 resectable cases of esophageal cancer were candidates for studing the histological effect reaching to primary lesion and lymph nodes, and effective oil BLM in metastatic lymph nodes, the following results was got. Moreover, the histological effect was defined as Ef1; no effect and slight effect, Ef2; moderate effect, Ef3; marked effect according to Carcinoma of the Esophagus Descriptive Rules in Clinic and Pathology. About the effect of preoperative combined therapy to primary lesion, it revealed that group IV, V were more effective than group II, III, especially in group V, 80% of cases were found to be effective. In Ef2, Ef3 cases of primary lesion, compared with 50% of metastatic rate of n1, 2 lymph nodes in group I, III, the group II; 33.3%, group IV; 21.4% and group V; 21.1 % showed that the metastatic rate were decreased in order. In cases of Ef3, group V showed lymph nodes free from metastasis highly up to 75%. The lymph nodes inside and outside the irradiation field were studied and although the effective rate found to be high in lymph nodes inside the irradiation field, but it could not be said that the effect was sufficient in lymph nodes outside the irradiation field. In comparison with the regular BLM, oil BLM maintained the high concentration of long period both in serum and lymph node, besides, even in clinical cases, the lymph nodes outside the irradiation field were recognized to be effective too.
