EXTRA-ANATOMICAL BYPASS VIA THE OBTURATOR FORAMEN IN TREATMENT FOR AN INFECTED VASCULAR PROSTHESIS IMPLANTED IN THE LEFT COMMON FEMORAL ARTERY

Reports and discussions concerning extra-anatomical bypass surgery via the obturator foramen have been limited, because of the special conditions of its indications.
A 65-year-old male, suffering from infection of a synthetic prosthesis implanted in the left common femoral artery after resection of an aneurysm, was successfully treated by resection of the infected prosthesis followed by the obturator foramen bypass. There were neither difficulties in the surgical procedure nor postoperative complications such as neurological disturbances, infection, excessive bending of the prosthesis, etc. The bypass prosthesis is patent 8 months after the operation.
It is suggested from this experience that the extraanatomical bypass via the obturator foramen may be primarily applied to arterial occlusive disease of the ilio-femoral region, instead of conventional revascularization.
