A pathological study was performed with regard to cancerous extension and nodal involvement using multi-sliced specimens to look for an appropriate operation with less intervention than radical mastectomy applying on the treatment of early cancer under 2.0cm in diameter.
Among sixty cases examined, 3 cases with non-infiltrating and 13 cases with infiltrating carcinomas extended beyond 2.6cm from the margin of the main lesion. The nipple was involved in 2 cases with non-infiltrating and 3 cases with infiltrating carcinomas located in non-areolar area. These facts indicate a probability to leave cancerous residue if a partial mastectomy and/or preservation of the nipple was performed. Frequency of the cases without metastasis in the regional lymph nodes were 81.5% in the tumor under 1.0cm in diameter and 64.3% in those of 1.1-2.0cm.
In conclusion, a complete dissection of regional lymph nodes should be performed in even such a small tumor in addition to a simple mastectomy.
