CURRENT TOPICS REGARDING PERIOPERATIVE INFECTION IN COLORECTAL SURGERY

The principle of prophylaxis for patients undergoing colorectal surgery should include antibiotics with activity against Enterobacteriaceae and Bacteroides fragilis group species. In addition, coverage of Pseudomonas aeruginosa, Enterobacter cloacae, and Enterococcus faecalis is recommended for empirical therapy in surgical site infections (SSIs). Because the prevalence of resistant organisms has been increasing, it is reasonable that the choice of prophylactic antibiotics and empirical antimicrobial therapy in patients with postoperative SSIs be modified in reference to contemporary antibiotic susceptibility data among common isolates from SSIs. The results of Japanese nationwide surveillance of the antimicrobial susceptibility of organisms isolated from SSIs have recently been published. To understand the results better, we carried out post-hoc analysis of the data to explore the antimicrobial susceptibility patterns of common organisms isolated from SSIs and suggested appropriate antibiotic prophylaxis and primary and alternative antibiotic therapies for patients with SSIs including postoperative intraabdominal infections with the specific intent of covering the common organisms isolated from SSIs in colorectal surgery.
