Surgical training is essential for ensuring the safety of advanced and complex operations. Training methods used for gastroenterological surgery are conventional On-the-job and Off-the-job training (Off-JT). Off-JT includes dry labs using simulators and models, wet labs and animal labs using experimental animals, and cadaver surgical training. In the field of gastroenterological surgery, Off-JT is used to certify the ability to perform robotic surgery properly and safely. Moreover, Off-JT is also conducted in dry and animal labs for training in endoscopic surgery and in animal and cadaver labs for simulation training in rarely occurring cases of acute-care surgery. However, Off-JT is not a common practice, since the completion of Off-JT is not required in the board certification process for gastroenterological surgeons in Japan. This article discusses the current status of Off-JT in the field of gastroenterological surgery and future perspectives.
