PATHOGENESIS OF CHRONIC ALCOHOLIC PANCREATITIS
A useful model of chronic alcoholic pancreatitis (CAP) is not available at present. We succeeded to produce a new canine model by the combination of alcohol administration and incomplete pancreatic duct obstruction. Histopathologically, the pancreas showed fibrosis with inflammatory cell infiltration, which is similar to that of human CAP. Thereforc, we propose a following new hypothesis. If the main pancreatic duct is obstructed and pancreas juice containing alcohol stagnates for a prolonged tirne, pancreatic parenchymal cell damage by alcohol may be markedly increased, which may result in the early formation of CAP (new big duct theory). We are able to explain why CAP occurs in only a limited nurber of heavy drinkers, and why CAP occurs infrequently with liver cirrhosis.
