COMBINED METHOD OF ANTI- AND RETROGRADE PERFUSION OF CARDIOPLEGIA
We have experimentally reported usefulness of the combined method of anti- and retrograde perfusion of cardioplegic solution in cases with coronary artery stenosis. In the present study, we have compared myocardial protective effect of this combined method (Retro-group, n=13) and antegrade perufison method alone (Ante-group, n=14) in patients who underwent coronary artery bypass gafting (CABG). Postopeative recoveries of cardiac function and rhythm and postoperative values of serum CK-MB and myoglobin were similar in the two groups. Additionally patients who underwent CABG to LAD using internal thoracic artery (ITA) in each group were selected and compared (Ante- and Retro-subgroup, n=6 in each). Postoperative cardiac index was singnificantly higher in Retro-subgruup (< 0.02) although there were no differences in CK-MB and myoglobin. Thus, the combined method of anti-and retrograde cardioplegia can afford optimal myocardial protection especially in patients in whom ITA is used.
