EMERGENCY AORTIC AND MITRAL VALVE REPLACEMENT IN ACTIVE PHASE OF INFECTIVE ENDOCARDITIS OF A 12-YEAR-OLD GIRL

This report describes a 12-year-old girl who contracted acute infective endocarditis and was cured by emergency aortic and mitral valve replacement during the active phase of the infection.
On May 20, 1981, the patient was admitted to this University Hospital with fever and severe congestive heart failure in functional Class IV. Two weeks before this admission, she noted the onset of malaise and dry cough on a background of good health. Following the infection, aortic and mitral regurgitation developed. On May 25, rapid clinical deterioration required emergency operation despite the presence of active infection. The aortic valve was replaced with Björk-Shiley Valve 19 ABP and the mitral valve with Björk-Shiley Valve 23 MBRC. The blood culture was negative. However, an acute inflammatory reaction was observed microscopically in the specimens of excised valves.
The patient has been followed for eight months since the operative procedure. Now, she is enjoying her school life in good health.
