EXPERIMENTAL STUDIES ON INJURIES TO THE MAIN PANCREATIC DUCT

The purpose of this study was to elucidate the healing process in various injuries to the main pancreatic duct in dogs. The survival time, causes of death, changes of serum amylase levels, pancreatography and morphological changes in the pancreas were observed.
When the main pancreatic duct was severed or incised longitudinally, the injuried duct was recanalized by an attachment to the greater omentum or neighbouring organs in many dogs, within a few days after surgery without severe complications or pathological changes in the distal pancreas.
When the main pancreatic duct was resected, the distal cut-end of the duct became obstructed later in most dogs by attaching itself to the surrounding organs, causing a marked atrophy of the distal parenchyma and severe illness with pancreatic functional insufficiency. However, no such complications were seen when a tube was inserted through the resected duct.
Hematoma, abscess, and pseudocyst were sometimes obseved following the pancreatic duct injuries. Furthermore, if the pancreatic duct was communicated to the hematoma or abscess, a pseudocyst was occasionally observed.
The serum amylase level elevated in all dogs after various injuries to the main pancreatic duct, and whenever the serum amylase elevation was either continuous or recurrent, development of pseudocyst should be considered.
When the main pancreatic duct injuries were associated with an injury of the papilla of Vater, severe pancreatitis developed.
