THE MANAGEMENT OF INTRAHEPATIC GALLSTONES

A series of 82 patients with intrahepatic gallstones seen at Nagasaki University Hospital have been analyzed and divided into two main groups, depending upon the combination of the site of intrahepatic galstones and the causative stenotic lesions of the biliary system. More than half (62%) of the cases of intrahepatic gallstones were combined with stenotic lesions of the intrahepatic biliary tree.
The anatomical and histological studies of these affected intrahepatic bile ducts were performed with regard to methods of treatment and postoperative clinical course and suggestion has been made that the intrahepatic gallstones with stenotic lesion in the intrahepatic biliary tract have been treated most effectively by hepatic lobectomy or segmental hepatectomy of the affected area with additional bile drainage via extended hepaticocholedochojejunostomy.
