STUDY ON RISK FACTORS OF POSTOPERATIVE DEEP VEIN THROMBOSIS
Three hundred patients undergoing major abdominal surgery and gynecological operations were investigated to discern risk factors of postoperative deep vein thrombosis (DVT). DVT was assessed by using
125I-fibrinogen uptake test. Coagulation and fibrinolytic examinations were simultaneously performed. Correlation between development of DVT and age, height, body weight, overweight, smoking, associated diabetes mellitus, presence of varicose vein, history of previous DVT, length of operation, bleeding volume during operation and serum lipid were also analysed. Following results were obtained.
1) Incidence of DVT was 13.3 per cent.
2) Over the age of fifty, overweight, associated diabetes mellitus and longer operative duration were significant as risk factors of development of DVT.
3) Free fatty acid was statistically higher in patients with DVT than those without DVT.
4) The presence of hypercoagulability before operation and on the first postoperative day, in addition to postoperative fibrinolytic shut down, was considered to be the most important factor to development of DVT.
5)
125I-fibrinogen uptake test should be indicated to the cases such as high risk group mentioned above in Japanese.
