A 19-year-old symptomatic male was found to have a obstruction of the second portion of the duodenum, medial displacement of the stenotic portion of the duodenum, and mucosa! effacement by barium X-ray studies. He underwent laparotomy under the diagnosis of duodenal stenosis, and an annular pancreas was found. Duodenojejunostomy and selective proximal vagotomy plus pyloroplasty (Holle's method) were carried out. Postoperatively he had a favorable course with Visick grading I and a decrease of the gastric acid secretion by more than 80%.
