EXPERIMENTAL STUDIES OF PANCREATIC FUNCTIONS BEFORE AND AFTER PANCREATICO-ENTEROSTOMY

Little is known about pancreatic endocrine and exocrine function after pancreatico-enterostomy in chronic pancreatitis. In order to study it, first, the large pancreatic duct of dog was ligated and chronic pancreatitis was produced. Second, pancreatico-duodenostomy was done 1,2 and 3 months after the ligation. Sequentially Pancreozymin-Secretin test and intravenous glucose tolerance test were studied every month.
The following results were obtained:
1) Pancreatic exocrine function falls gradually and is abandoned finally after the ligation. The earlier pancreaticoduodenostomy is done, the better pancreatic exocrine function is maintained.
2) Pancreatic endocrine function is kept normal if the anastomosis is done while some pancreatic exocrine function continues.
3) Dilatation of the pancreatic duct is reduced by the anastomosis.
4) Histologically, fibrosis of the pancreas is restrained and inflammatory cell infiltration is disappeared.
These results suggest that early operative intervention should be done as a treatment of chronic pancreatitis.
