The National Clinical Database (NCD), established in 2011, has confirmed the diversity of clinical practice in vascular surgery, which comprises surgical as well as conservative therapy for arterial, venous, and lymphatic conditions throughout the body. To evaluate the quality of medical practice in aortic surgery, risk-adjusted surgical mortality rates can be used as an indicator. However, in most of the wide variety of other vascular procedures with low mortality rates, follow-up of patients and the collection of data or surrogate markers for quality of life are indispensable for the assessment of the quality of treatment. In 2013, the Japanese Society for Vascular Surgery started a project on nationwide registration and a tracking database for patients with critical limb ischemia who are treated by vascular surgeons. This database, called the JAPAN Critical Limb Ischemia Database, was created within the NCD. Although still not in the stage of contributing to improved quality of medical care, a general overview of this project is reviewed. A clinical study using the data accumulated in the NCD to achieve improvement in the quality of medical practice in vascular surgery is also reviewed.
