RECURRENCE OF COMMON BILE DUCT STONES IN PATIENTS WITH CONGENITAL CYSTIC DILATATION OF THE COMMON BILE DUCT

It was considered that congenital cystic dilatation of the common bile duct with fusiform or cylindrical dilatation was liable to cause stone formation in the common bile duct. We had 784 patients with biliary diseases excluding malignant biliary tumors in this 15 years, and they consisted of 264 patients with congenital cystic dilatation of the common bile duct and 520 patients without the malformation. Of the 264, 248 patients showed fusiform or cylindrical dilatation of the common bile duct, and 64 of the 248 had reoperation for recurrence of common bile duct stones (29 patients), residual stones (18), stones unclear in genesis (5), repeated attacks in pancreatitis (6) and cholangitis due to stricture of the stoma of biliodigestive anastomosis (6). The 29 patients with recurrent bile duct stones had cholecystectomy with choledocholithotomy or with T-tube insertion alone. On the other hand, none of patients with any bypass procedures to the biliary tract had recurrence of bile duct stones. The 29 patients had 71 times of the explorations, 3 had 4 times of the explorations, 7 had 3 times and 19 had 2 times. The interval of recurrence of stone formation in the common bile ductis in over 6 months to 17 years and there was the highest incidence of recurrence of bile duct stone in 3 to 5 years.
