NON-OPERATIVE ESOPHAGEAL RECONSTRUCTION OF CONGENITAL ESOPHAGEAL ATRESIA

The so-called Rehbein's silver olive method was proposed in 1971 for reconstruction of esophagus as an alternative procedure of bowel subtitution in congenital esophageal atresia with a wide gap. We developed a new instrument in order to perform Rehbein's method without thoracotomy. Experimental esophageal atresia was produced in four dogs. Eight weeks postoperatively a nylon thread was passed between esophageal pouches with the instrument inserted through an oropharynx and a gastrostomy, thus avoiding thoracotomy. Thereafter the animals were successfully treated according to Rehbein's silver olive method. Further, the instrument was successfully applied to 3 of 4 infants with congenital esophageal atresia and 2 infants with esophageal stricture.
The reconstructed esophagus was functionally satisfactory in all of them. In cases with a short gap between esophageal pouches, reconstruction of the esophagus will be easily achieved by this instrument without thoracotomy.
