TRANSITIONAL CARE FOR CONGENITAL HEART DISEASE

Advances in the field of congenital heart disease (CHD) surgery have increased the number of adult CHD patients, now exceeding 500,000 in Japan. While patients with simple CHD can lead unrestricted lives, those with complex CHD often develop a variety of late complications and require continuous specialized care. In particular, cyanotic and single-ventricle diseases frequently involve multiple organ dysfunction, necessitating expert treatment.
Transitional care for CHD is mainly led by pediatric cardiologists and cardiologists. Eight related medical academic societies issued “Recommendations for transitional care for congenital heart disease to adulthood” and recommended a multidisciplinary team approach. The transition process may vary from institution to institution. Education of CHD patients using a transition readiness checklist is also recommended.
However, many issues remain, including 1) patient education, 2) pregnancy and delivery management, 3) transition of patients with complex CHD, 4) transition of patients with cognitive dysfunction, and 5) transition of patients with multiple noncardiac diseases. While transitional care for CHD patients is relatively advanced, further system enhancements, professional training, multidisciplinary collaboration, and societal support are all essential for sustainable progress.
