In spite of world-wide remarkable progress in heart transplantation in recent years, it has not been performed in Japan for over 20 years. One of the reasons is the problem related to the brain death. However, there seems increasing trends to accept brain death and to promote organ donation in general, and it is quite important to organize the heart transplant program supported at least by an adequate regional network system. In the basic research, efforts should be made for improvements in organ preservation, postoperative management for immunosuppression and infection. In addition, diagnosis of brain death in small children should be established in the future.
