PROBLEMS RELATED TO RESUMING CLINICAL LIVER TRANSPLANTATION

The Committee For Medical Ethics at our institution gave consent to our proposal to perform liver transplantation from brain-death donor in February 1990. This report summarizes our preparations for accepting the approval from the committee.
The preparations for this involves four different areas. The 1st is related to direct recipient surgery. Surgical apparatus including Bio-Pump, Rapid lnfusion System, and other instruments has been prepared. Nine of the 13 members of the team as well as an anesthetist and two nurses have received training of perioperative care for liver transplants in large centers in USA and/or Europe.
The 2nd necessity is establishment of an organizing committee, mainly consisting of internists for selection of potential recipients.
The 3rd necessity is establishment of an assistance system for liver transplant recipients. We have already had experience of clinical immunosuppression in 260 renal allotransplants and 100 bone marrow transplants. For understanding of all staffs, We have compiled a transplant manual and a nursing manual.
The 4th necessity is establishment of a donor supply system, which has not yet been achieved. By keeping constant direct contact with a number of institutions which have emergency and neurosurgery units, a supply of livers will be obtainable.
