SIGNIFICANCE OF MULTIDISCIPLINARY THERAPY FOR HEPATOCELLULR CARCINOMA

The significance of multidisciplinary therapy for hepatocellular carcinoma was investigated in 121 resected cases.
1) The 5-year survival rate was 100% in absolute curative resection (12 cases), 59.1% in relative curative resection (37) and 10.9% in relative non-curative resection (59). However, no cases surviving more than 3 years were observed in absolute non-curative resection (13).
2) The non-recurrent rates were not so different between preoperative TAE(+) and TAE(-) groups in absolute and relative curative resection, although 1- and 3-year non-recurrent rates were 92.3% and 53.8% in relative non-curative resection with preoperative TAE, 56.1% and 28.1% in relative noncurative without preoperative TAE, showing effect of preoperative TAE in relative non-curative resection.
3) Functional disturbances of coagulation-fibrinolysis system in cirrhotic patients were improved after PSE.
4) Treatments for the recurrent HCC : a) The 1-, 2- and 3-year survival rates after TAE were 79.0%, 42.0% and 9.0%. b) The 1-, 2- and 3-year survival rates after the reoperation were 100%, 75.0% and 25.0%. c) Three cases with the recurrent HCC were effectively treated by continuous intraarterial infusion of 5-Fu and lentinan with intermittent one shot injection of ADR using a subcutaneous infusion pump. Partial response (PR) was observed in 2 cases. All patients have been still alive 1 year 7 months, 1 year 4 months and 6 months after the treatment.
