PROBLEMS IN INTERNATIONALIZATION OF JAPANESE GENERAL RULE FOR THE TREATMENT OF LARGE BOWEL CANCER

Surgical Department, Tokyo Metropolitan Komagome Hospital, Tokyo, Japan

Takashi Takahashi, M.D.

The Guidelines for the treatment of large bowel cancer in several countries are discussed on each aspect which compose staging, general assessment of description and histological classification.
Japanese General Rules have many distinctive assessments for intra- and post-operative features of large bowel cancer, which are based on over stressing on lymph node dissection. As for grouping and grading of the regional lymph nodes for both colonic and rectal cancer, Japanese Rules are theoretical but a little impractical, which also reflect on a complex classification for the radicality of operation.
In order to generalize the Japanese Rules worldwide, a further understanding of the idea and technique of lymph node dissection will be inevitable among foreign doctors and for Japanese side some modifications on self-complacent documentation form for several features of large bowel cancer will be necessary.
