The first edition of the general rules for the description of thyroid cancer was published in 1977 referring TNM classification by UICC. The rules decided details to be described before, during, and after operation. The most important determination was the items of size of tumor and location of swelled lymph nodes which have not been described in the TNM classification. Since then, the size of tumor has been emphasized to be accepted on the TNM classification by Japanese Joint Committee (JJC) at the UICC meeting. In the second edition of the general rules published in 1983, it was determined that T category should be classified by tumor size, 2 and 5cm. After 2 years, however, UICC published a revised edition in which T category was classified by 1 and 4cm. At this time, difference between the two classifications has newly developed. To make clear the significance of the difference between the two, both T categories were compared with survival and recurrence rates. The study revealed no significant difference, and JJC decided not to push UICC from this point.
