Since 1977 we have experienced 287 neonatal surgical cases in Tsukuba University. Mortal cases within 1 month were 25 (8.4%) and mortal cases thereafter up to the present were 12 (4.0%). Cause of death was mainly stemed to severe chromosomal anomalies such as 13q-, 18 trisomy and 21 trisomy, and various major cardio-vascular anomalies. Six 18 trisomy were treated according to our own idea. Operative results of esophageal atresia and omphalocele were inclined to worse side due to this attitude. Seven out of 9 cases of 21 trisomy were mortal because of severe cardio-vascular anomalies.
Prenatal diagnosis has been greatly valued in recent years. The clinical value of real-time echogram has been established for detecting various fetal anomalies. In 103 echographically diagnosed cases, most of them were related to surgical disease categoly. There were 11 cases, which were successfully treated with surgical intervention. These included one case of CCAM and one 1,000g case with gigant sacrococcygeal teratoma bleeding massively after Caesarian section.
There were 7 aborted cases within 22 weeks.
There were 8 cases, born without preper diagnosis of pediatric surgical diseases. Future progress for promotion of better diagnostic ability and diagnostic team work is expected.
