MANAGEMENT OF CRITICALLY ILL PATIENTS WITH CONGENITAL DIAPHRAGAMTIC HERNIA

Forty-two patients with congenital diaphragmatic hernia who were admitted before 24 hours old were reviewed. Persistent fetal circulation (PFC) was the most serious pathological condition and mainly contributed to produce the hypoxia. In order to overcome the condition of PFC, the alkalosis therapy induced by hyperventilation, and the pharmacological treatment using tolazoline were firstly selected. If the patients were obstinately resistant to these therapies, the extracorporeal membrane oxygenation was employed. These therapeutic steps were successful, and no one died of PFC in 21 patients over the past 5 years.
