EXTENDED SURGERY FOR ADVANCED CARCINOMA OF THE GALLBLADDER

Results of extended surgery for advanced carcinoma of the gallbladder were reviewed. Extended right hepatic lobectomy (EHR) was performed in three patients and pancreatoduodenectomy (PD) in 13. One patient underwent both EHR and PD (HPD). There was no operative death nor hospital death. Both of two patients with stage III disease survived longer than 4 years. On the other hand, none of 13 patients in stage IV survived 3 years. In eight of 13 patients with stage IV disease, operations turned out to be non-curative mainly because of remaining cancerous tissue at the surgical margin. To perform curative surgery for advanced carcinoma infiltrating into hepatic hilum, more aggressive approach including HPD seemed necessary. HPD must be indicated when surgery is highly likely to be curative as the quality of life after HPD is still unsatisfactory.
