One hundred and twenty-nine cases of carcinoma of the gallbladder were reviewed to discuss how to treat advanced carcinoma of this organ. Among 129 cases, 62 were resected, but only 32 of them were considered to be curative. Twenty two of 32 curative resected cases were more advanced than stage II. Of 22 curative resected cases, more than 1 year survival was seen in 8 cases and 5 year suvival was seen in only one case. The surgical results of advanced carcinoma of the gallbladder were very poor as above mentioned. Some points were enumerated as problems of the radical operation for advanced carcinoma of this organ; extensive dissection around hilar part, invasion to the hepatoduodenal ligament, retropancreatic space and retroperitoneum, and combined resection of adjacent organs. Complete resection of the hepatoduodenal ligament was considered to be a coming important subject.
