THE RESULT AFTER ILEOANAL ANASTOMOSIS

1) Among patients with adenomatosis coli (AC, 37 cases) and ulcerative colitis (UC, 17 cases) in whom total colectomy, mucosal proctectomy and anastomosis was performed, 38 cases with the closure of diverting ileostomy were retrospectively studied. Their reservoir type was J-pouch in 28 cases, Non-pouch in 8 cases and H-pouch 2 cases. The follow up period ranged from 1 month to 8 year and 6 months (mean 5.6 years).
2) The stool frequency in patients with AC and UC was 5.1 and 7.5 times daily, respectively.
3) The result by Peck's criteria was as follows : Excellent in 21 cases (AC17, UC4), good in 14 cases (AC11, UC3), and fair in 3 cases (UC) and failure in 6 cases (AC). The outcome had a good correlation ship with maximaum anal static pressure.
4) Late complication included ileus (12 cases), ureteral stone (7 cases), desmoid (7 cases in AC), anemia (7 cases) and pouchitis (2 cases).
5) The improvement in frequency of pelvic sepsis, operative time and bleeding volume is attributed to ano-abdominal approach.
6) This procedure is considered beneficial in patients with AC or UC who might require proctocolectomy.
