TREATMENT OF GASTROINTESTINAL CARCINOIDS WITH SPECIAL REFER-ENCE TO MALIGNANT CHARACTERISTICS

Malignant features of gastrointestinal carcinoids were evaluated from 3 different viewpoints : (A) Invasive nature on histologic examination ; (B) Statistical analysis of metastases in the Japanese series of reported cases ; and (C) Malignant features of early gastric carcinoids in a rodent, Mastomys. (A) Invasive nature was evident in 99.3% (133/134) of carcinoids in the digestive organs. (B) Of 928 carcinoids in the digestive organs, 27.8% were reported to have metastases most often to lymph nodes ; hematogenous metastases also showed a high incidence particularly in cases with rectal carcinoids. A comparative analysis was carried out between 336 sm-gastrointestinal carcinoids and 198 sm-gastric carcinomas ; among cases with the size 2.0cm or smaller, the incidence of metastases was 9.6% and 8.1%, respectively, and among those with the size greater than 2.0cm, it was 30.2% and 28.6, respectively. (C) The early carcinoid formation in the base of Mastomys’ gastric glands was often associated with invasive neoplastic nests in the neighboring stroma of the lower mucosa.
It was concluded that gastrointestinal carcinoids should be treated as a malignant neoplasm in accordance with the policy for treating ordinary carcinomas in the gastrointestinal tract.
