SURGICAL STRATEGY FOR INSURINOMA

The findings in twenty-four patients with insulinoma were analysed to establish preoperative localizatin of the tumor. The incidence of patients with multiple tumors and malignancy were 12 per cent (3/24) and 29 per cent (7/24), respectively. Superselective arteriography and percutaneous transhepatic portal vein catheterizatin are highly recommended approaches. Recent use of intraoperative ultrasonography increases the detection of tumor location of occult tumors which locate deeply in the head of the pancreas. At the time of surgical intervention, a cautious exploration of the pancreas after thorough mobilization is the most important. Enucleation can be done when intraoperative findings of the tumor and regional lymph node indicate no malignant features and non multiple legions. Regarding to the difficulty of the histological diagnosis of malignancy, we stress the possibility of differential diagnosis by means of DNA histogram of the tumor using Feulgen stain.
