FUNCTIONAL ASPECT OF THYROID SURGERY : GRAVES' DISEASE

Post-operative thyroid status was examined in 136 patients with Graves' disease for two years, aiming to determine the duration of postoperative transient hypothyroidism. At the 4th month after surgery 29 patients or 21.3% were hypothyroid, however, by the end of the first post-operative year only 6 patients remained hypothyroid. Long-term post-operative thyroid status was examined in 325 surgical patients 4 to 13 years after operation. In 25 patients (7.7%) recurrent hyperthyroidism was observed. Two hundred and twenty eight patients (70.1%) were euthyroid and 53 patients (16.3%) had raised TSH with normal thyroid hormone levels. Nineteen patients (5.8%) were hypothyroid. Thyroid function was re-examined 18 months after the first review in 189 patients who were not under treatment. An upward change in thyroid function was observed in 26 patients (14%), whereas downward change was observed in 12 patients (6%). Thus post-operative thyroid status in some patients must be recognized unstable and may fluctuate.
