STRATEGY AGAINST ESOPHAGEAL CANCERS ON THE BASIS OF CELLULAR BIOLOGICAL CHARACTERISTICS

Even in cases in which tumors were microscopically shown to have invaded organs, the prognosis of a3 without node metastasis is unexpectedly favourable. Moreover, the five-year survival rate of patients having superficial cancers without node metastasis (26 cases) is 94%, but that of patients with cancers with node mateastasis (17 cases) is 39.4%. It is suggested that this dominant different prognosis is due to cellular biological characteristics and/or molecular biological properties. Thus the following fundamental studies have been undertaken.
1) Sialic acid, sialidase, sialyltransferase : The content of sialic acid was correlated to the activity of sialyltransferase, but was in opposition to sialidase activity. One patient with node positive superficial cancer and a high level of activity of sialyltransferase developed multiple node and liver metastasis.
2) Glutathione S-transferase-π (GST-π) : The content of GST-π in cancer cells is the most abundant among normal esophageal epithelial cells, displastic cells and cancer cells. Moreover, cells with rich GST-π show greater resistance to anticancer agents than those with poor GST-π.
3) DNA histogram: The two-year survival rate of diploid tumors is 92.3% and that of non-diploid tumors is 49.4%. The DNA index of tumors with skip lesions is shown to be over 1.5, and such cases wereall non-diploidy.
4) Expression of EGFR of cancer cells : EGF content was detected with 125I-hEGF binding assay. EGF binding capacity (fmol/mg protein) is the highest on metastatic nodes and higher on primary lesions of cancer than on normal esophageal mucosa.
5) Subtypes of tumor infiltrating lymphocytes (TIL) : The majority of infiltrating cells around the tumor tissues analysed with monoclonal antibodies were demonstrated to be T-cells. However, Helper/Inducer and/or Killer/Suppressor cells were equally detected.
6) Aggressive chemotherapy after curative operation of cancer of the esophagus with distant node metastasis : The five-year survival rate of the ten patients undergoing the above treatment was 30%, higher than that of all other types of therapies.
