PROBLEMS OF HEPATIC RESECTION FOR HEPATOCELLULAR CARCINOMA

First Surgical Department, University of Tokyo, Tokyo, Japan

Takeshi Nagao, Nobuhiro Kawano

We summarized our experiences with hepatic resections for hepatocellular carcinoma. Ninety-eight hepatic resections were performed upon 94 patients until the end of 1985. The followings are our concIusions.
1. Operative blood loss is one of the critical factors that decide the operative results.
2. The size and number of tumor and the existence of tumor capsule are important factors that influence the long-terrn prognosis.
3. Segmentectomy seems to be a suitable operation for hepatocellular carcinoma less than or equal to 5cm in maximal diameter.
4. The prognosis of small hepatocellular carcinoma less than or equal to 2cm was fair, and local excision may be acceptable for this small tumor.
