REMOTE RESULTS OF SELECTIVE VAGOTOMY WITH ANTRECTOMY FOR PEPTIC ULCER AND PROSPECT OF THE TREATMENT

Purpose:This study was designed to compare the results of selective vagotomy with those of antrectomy (SV + A), which has been performed in our department for peptic ulcer, with those of extensive gastrectomy (EG) or antrectomy alone (A).
Subjects and Methods:Subjects were 195 undergoing SV + A, 47A, and 245 EG. The examination of gastric juice and barium X-ray studies were carried out.
Results:The acid reduction rate 1 year after operation was greatest in group SV + A for both BAO and MAO (100 and 97%, respectively) followed by group EG (88.5% for MAO). Postoperative retention of gastric contents, a drawback of SV + A, was observed in 17.4%. The operative death rates were 0% for SV + A and 1.2% for EG. The recurrence rates were 0% for SV + A and 1.2% for EG. The recurrence rate was 0% for SV + A, 0.3% for EG, and 6.0% for A.
Discussion and Conclusions : These results indicate that SV + A is more satisfactory in terms of the acid reduction rate and the recurrence rate than the other procedures but is more often associated with postoperative retention of gastric contents. The dogs that underwent SPV + mucosal antrectomy showed favorable acid reduction and stomach emptying, suggesting that this procedure is more advantageous than SV + A.
