REVASCULARIZATION METHODS AND THEIR LATE RESULTS IN CHRONIC INTESTINAL ISCHEMIA

Eleven patients with chronic intestinal ischemia underwent various types of surgical revascularization during the past 22 years. In this paper, a comparison of the late results according to the types of revascularization method is presented. Retrograde aorto-superior mesenteric prosthetic grafts inserted to 4 patients revealed no late occlusion, but in one patients, an anastomotic aneurysm complicated with fistula to the duodenum occurred 7 years after operation.
Retrograde aorto-superior mesenteric artery vein bypass resulted in a late recurrence rate of 75% (3/4). Over all recurrence rate was 40%.
Because of high recurrence rate, these retrograde bypass have been abondoned as revascularization method in chronic visceral ischemia since 1983. In alternation, we have employed antegrade aorto-lienosuperior mesenteric artery bypass using pantaloon vein graft.
We have used this technique upon the recent two patients including one for reoperation and have had no complication. No graft failure have occurred.
We believe that this technique is advisable in order to obtain the better long term patency.
