APPLICATION OF VASCULAR SURGICAL TECHNIQUES TO THE OPERATION IN GENERAL SURGERY

The 2nd Department of Surgery, Hokkaido University School of Medicine, Sapporo, Japan

Tatsuzo Tanabe

Necessity for management of intraoperative vascular problems and applied surgical techniques for general surgeons has been increasing. Based on extensive experiences of various reconstructive surgery in 1200 cases of vascular diseases, we have tried to extend these vascular surgical techniques to the operation in general surgery. Shunt operation for portal hypertension, repair of vascular injury during operation and extended combined resection of various cancers were the major applications. Those experiences and operative results were reviewed. Although these vascular surgical techniques can be widely applied in general surgery, combined resection for various cancers infiltrated into the vascular wall should be limited. Application of combined resection may increase the resectability of cancer, but apparent increase of the survival rate seems to be not yet demonstrable. Strict indication, complication and exact surgical techniques should be clarified with further careful analysis of the follow-up study.
