MITRAL VALVE SURGERY IN PATIENTS OVER 40 YEARS. RISK FACTORS INFLUENCING EARLY DEATH

A series of 120 patients over 40 y.o. who were operated on for mitral valve repair were included in this study. About 70 different factors were investigated, and several risk factors with regard to early mortality caused by LOS (low cardiac output syndrome) were revealed. Significant risk factors in early mortem LOS patients compared with surviving patients were found to be as follows :Over 70% CTR, over 50mm LVDs (left ventricular systolic dimension), over 60mm LVDd (left ventricular diastolic dimension), and under 40% left ventricular ejection fraction. Additional risk factors were found to be as follows : Inadequate myocardial protection during cardiopulmonary bypass (CPB), under 90mmHg systolic blood pressure at cessation of CPB, over 18mmHg mean pulmonary wedge pressure with under 2.0L/min/m2 cardiac index, over 50I.U. CPK-MB values, over 80mg/dl lactic acid, and myocardial lactate production after 6 hrs of cessation of CPB.
