PATHOBIOLOGIC FEATURES OF GASTRIC CARCINOMA AND SURGICAL TREATMENT IN THE AGED PATIENTS OVER 70 YEARS OLD

Two hundred and forty two cases of gastric carcinoma in the patients over 70 years old were analyzed in terms of pathologic features and results of surgical treatment. Carcinoma in the patients over 80 years old was characterized by the location in the lower part of the stomach, differentiated carcinoma and low grade advancement in depth invasion and lymph node metastasis, and better outcome after less aggressive operation such as R0 or R1. In contrast, carcinoma in those between 70 and 79 years old spread more extensively to the lymph nodes, liver and peritoneum and resulted in poor outcome after surgery. Limited operation may be permitted for the former, but extended radical operation and adjuvant cancer chemotherapy should be applied for the latter if possible.
