URINARY AND SEXUAL DYSFUNCTION AND THE MODE OF RADICAL OPERATION FOR RECTAL CANCER

Postoperative urinary and/or sexual function was evaluated in 215 patients on whom a radical operation for rectal cancer was performed more than 6 months before.
Urinary dysfunction occurred in 101 cases out of total 215 cases (47%). In male of 123 cases, erection disturbance was experienced in 53 cases (43.1%), whereas ejaculation impairment was seen in 80 cases (67.2%).
There were seen some differences in incidence of these disturbances according to its mode of operation, namely, urinary dysfunction rate of 68.4% and erection disturbance rate of 58.5% was obtained in the group which underwent Miles (abdominoperineal resection) operations. On the other hand, in anterior resection group, urinary dysfunction occurred in 26.8%, erection disturbance in 27.6% which show the better results than those of in Miles group.
Incidence of ejaculation impairment was unexpectedly high, however, both in Miles group (81.1%) and in anterior resection group (67.2%).
In the consequence of recently performed extended excision including lateral dissection, grave damage on these functions was experienced, i.e., uritary dysfunction in 73.6%, erection disturbance in 86.3% and ejaculation impairment in 96%.
It is still a inevitable and antinomic problem to pursue the extended radical surgery and to avoid such functional disturbance.
