LATE DEVELOPMENT OF CONDUCTION DISTURBANCES AND ONSET OF VENTRICULAR ARRYTHMIAS AFTER CORRECTION OF TETRALOGY OF FALLOT

We studied the relation between the late development of conduction disturbances and onset of ventricular arrythmias related to postoperative hemodynamic state in 62 patients 10-16 years after repair of tetralogy of Fallot.
The patients with normal ventricular conduction were observed in only 8.3% of all. The disturbance of proximal conduction system was suggested in 30.0% of patients, but we did not encountered the late development to complete A-V block.
The late onset of ventricular arrythmias was observed in 30.7% of patients, 10 years or more after surgery. Also, we encountered the ventricular tachy-arrythmias in three patients: One with severe tricuspid regurgitation are treated medically, one with residual shunt and pulmonary stenosis was reoperated, and another with moderate residual stenosis was dead suddenly during sleep.
The progression of conduction disturbances was not related to postoperative hemodynamic state, but onset of ventricular arrhythmias was encountered in hemodynamically poor patients.
In conclusion, this fact suggests that it is very important to repair the lesions perfectly and to preserve the whole conduction system.
