HIGH DOSE CYCLOPHOSPHAMIDE TREATMENT AND SECOND LOOK OPERATION FOR PATIENTS WITH THE STAGE III AND IV NEUROBLASTOMA

As aggressive treatments, preoperative high dose cyclophosphamide treatment (CPM) and the second look operation (SLO) have been performed on 18 neuroblastoma patients with a less-than 50% cure rate. The ages ranged from 2 to 13 years except for a 11-month-old boy.
Eleven patients underwent CPM treatment but SLO were not performed. The mean survival period was prolonged over 7 months compared with those in the former patients. The main cause of the recurrence was the remnant of the metastatic regional lymph nodes.
Five patients underwent CPM treatment and second look operation 4 to 6 months after the primary operation. The regional lymph nodes were dissected completely in 2 patients with the stage III who are surviving without an evidence of disease. However, the stage IV patients who had neither lymph node metastases nor the primary tumor recurrence at SLO showed bone metastases after 6 months.
Therefore the treatment improved the cure rate in the patients with the stage III and prolonged the survival time of the stage IV neuroblastoma.
