STUDIES IN INTESTINAL PHASE OF GASTRIC SECRETION

The 3rd Department of Surgery, The University of Tokyo School of Medicin

Osamu Ihara, M.D.

To elucidate the role and mechanism of intestinal phase in gastric secretion, following studies were carried out.
Experiment I : Participation of intestinal phase in gastric secretion was studied in dogs with a Heidenhain pouch and a baloon catheter duodenostomy. Diversion of the test meal into the stomach and duodenum was achieved by manupulation of the baloon, thus ennable us to separate gastric and intestinal phases of acid secretion in a relatively physiologic condition. In addition, role of endogenous gastrin as a mediator of this process was investigated.
Experiment II : Action of amino acid as a chemical stimulating factor in intestinal phase was compared between parenteral and enteral administrations in dogs with duodenal fistula. Then, the efficacy was also studied in dogs with abdominal truncal vagotomy and/or abdominal evisceration, in which gastrointestinal hormones were completely deprived by removal of the entire digestive organs in the abdomen except the liver and gastric corpus.
Results and Conclusions : 1) The participation ratio of intestinal phase in digestive gastric secretion was as much as 1/3 of gastric phase. 2) A sort of interaction was seen between gastric phase and intestinal phase. 3) There was a latent period before the manifestation of response in intestinal phase. 4) The principal mediator of intestinal phase is considered to be intestinal gastrin and amino acid. 5) In intestinal phase amino acid stimulates the secretion of gastric acid in corporation with vagal, nerve, and without intermediation of gastrointestinal hormones.
