A STUDY ON THE HEALING PROCESS OF PANCREATICO-ENTEROSTOMY

A major problem of the pancreatico-enterostomy was the high incidence of leakage, however, still little is known about the exact healing process. The basic experimental study on the healing process of the pancreaticoenterostomy in 72 mongrel dogs was performed, comparing normal pancreas with experimental pancreatitis.
The postoperative findings were evaluated in histology, microangiography and tensile strength.
The results were obtained as follows;
1. While the incidence of leakage was 20% in normal pancreas group, it was 3.2% in pancreatitis group.
2. On the third day after operation, the anastomosed portion was found to have the dead space with bloody exudate in the normal pancreas group, but any dead space was not found in the pancreatitis group.
3. The tensile strength on the seventh day after operation showed stronger tension in the normal pancreas group than that in the pancreatitis group.
4. On the fourteenth and twenty eighth days after operation, no difference was seen histologically and microangiographically in the healing process between the two groups.
These results indicate that the firm and fixed anastomosis of pancreatic duct to the intestinal mucosa is the best to prevent major leakage. In addition, a special consideration should be taken on the anastomosed portion not to make dead space between the pancreatic stump and the intestine.
