SURGICAL TREATMENT OF ACUTE ARTERIAL OCCLUSION

During past 15 year, 184 patients with acute arterial occlusion were seen in the 1st Dept. of Surg., Univ. of Tokyo. Among them, 65 cases of embolism and 72 cases of thrombosis from acute exacervation of chronic arterial obstruction were studied.
Reconstructive operation were carried out in 40 cases (67.8%) for embolism and in 28 (38.9%) for thrombosis. Five cases (12.5%) in the former and 13 (46.4%) in the latter were amputated in legs.
The reconstructive operation for embolism seemed to be superior to that for thrombosis. However, the results of the operation for thrombosis were better than that of conservative therapy: Amputation rate was 68.2% in the latter cases. Moreover the limb salvage was obtained in 72.7% for the former case in recent 5 years.
To improve the results of reconstructive operation for acute arterial occlusion, the following precoedures would be important, 1) early and prompt operation, 2) correct and complete operation, 3) pre-and postoperative treatment including antithrombotic and anticoagulant therapy.
The amputation should be left for later with the hope of developement of collateral circulation.
