ON THE PROBLEMS ASSOCIATED WITH ACUTE ARTERIAL OCCLUSION

The authors investigated 58 cases of acute arterial occlusion due to arteriosclerosis, 18 of which were treated conservatively and 40 surgically with a success rate of 72% and 60%, respectively. The mortality was 26% (15 cases), the myonephropathic-metabolic syndrome (MNMS) en-countered in 6 cases being the principal cause of death. Examination of the ischaemic blood matter performed in 29 cases revealed rhabdomyolysis, nephrophathy or metabolic acidosis in 9 cases (31%). The mortality reached 56% (5 cases) in this group and all cases were of the disseminated type.
Experimental research on MNMS in adult mongrel dogs consisted of examination of the systemic blood, of the venous blood and of muscles in the involved limb, autopsy and pathological examination. Excellent results were obtained with wash-out of the venous blood in the involved limb and administration of adrenal-cortical hormones to treat the oedema.
The onset of MNMS transforms acute arterial occlusion from a regional disorder into a systemic one and an exact picture of the condition is of the greatest importance for the treatment.
