RECONSTRUCTION OF TRACHEAL CARINA WITH NEVILLE'S PROSTHESIS

The primary anastomosis is the best way to reconstruct the trachea and carina. When tracheal lesion is very great and malignant, we must resect the trachea widely and/or the carina. In such a case the primary anastomosis is very difficult technically. Although the reconstruction of the carina is very much difficult in any way,it is easier to reconstruct it with Neville's prosthesis.
Three cases with malignancy extending to the carina reconstructed with Neville's prosthesis were reported.
Two cases are alive and well for more than one year after operation. The results are very satisfactory. The postoperative management is also easy, since the laryngeal release and the postoperative anteriorly-forced flexion of the head are usually unnecessary.
As Neville's tracheal prostheses are not yet ideal, we must make efforts to develop the tracheal prosthesis.
