SUCCESSFUL TREATMENT OF A RIGHT HEPATIC ARTERY ANEURYSM ASSOCIATED WITH HEMOBILIA
We have experienced a case of aneurysm of the right accessory hepatic artery which occurred after cholecystectomy and was diagnosed by ultrasonography and selective angiography. Ligation of the involved artery was done and patient's postoperative course was good. Including this, 120 cases of hepatic artery aneurysm in the English and Japanese literature since 1960 are gathered and reviewed. The hepatic artery aneurysm caused by biliary surgery tends to locate at the right hepatic artery and rupture into the common bile duct more than 2 weeks after operation. Excision of the aneurysm and vascular reconstruction is difficult because of the strong inflammation and adhesion. Ultrasonogram is very useful for the diagnosis of hepatic artery aneurysm. If there is no factor to decrease the blood supply to the liver such as shock, anoxia, or portal thrombosis, ligation of the proper hepatic artery is not so danegerous as to have been mentioned previously.
