MICROANGIOGRAPHIC STUDY OF COLORECTAL CENCERS

Sixty-nine cases with colorectal cancers which received an operation for 5 years from April, 1975 to March, 1980 in our department were used. The microangiographic patterns of the tumors were compared with these of histopathological findings.
1. The normal vascular patterns were described. The vascular patterns of malignant lesions were classified into 3 groups: (1) changes of vascular lumen, (2) changes of the directions of vessels, (3) neovascularization.
2. The vascular patterns of hyperplastic polyp and adenomatous polyp resembled the normal mucosa. However, in foci of atypical glandular proliferation within a polyp, the regular arrangement of the vessels had become disorganized.
3. The polypoid cancer had long straight vessels appearing even straighter than the adenoma. Of early elevated cancer (IIa type), the sm cancer presented hypovascular pattern, and the m cancer hypervascular.
4. The advanced cancer of 1 type showed hypervascular patterns and the vascular disarray withvessels of varied size which histological foci of necrosis were often seen. The vascular patterns of 2 and 3 type were classified into 5 subtypes, and has been discussed with degree of cancer invasion, location of tumor and tumor site of lumen.
5. The carcinoma with liver metastasis had vascular disarray with histological foci of necrosis and the changes of vascularity in outside of tumor were recognized.
6. The findings obtained with microangiography correlated well with the histological observations with special reference to interstitial reaction of the carcinoma.
