DIAGNOSIS AND TREATMENT OF ESOPHAGEAL CANCER IN CHINA

Wu Tung

Trend in the diagnosis and treatment of carcinoma of the esophagus in China was reported. Incidence of carcinoma of the esophagus is very high, especially in North China. In one series from the County Hospital of Linhsien of Honan Province, the incidence of the esophageal cancer of 379, and the death rate of 100 among 100,000 populations, and it consituted the 1/5 of total deaths were reported. Cytological examination of abrasive balloon was commonly used for the early diagnosis and positively found in the 87.8-94.2% of patients. Left posterolateral thoracotomy and a one-stage extensive esophagectomy followed by esophagogastrostomy in the posterior mediastinum was commonly employed. Resectability rate of 75-85%, operative mortality rate of less than 5% and 5 year-survival rate of 30-40% were reported in the recent study. At the County Hospital of Linhsien, frequent population surveys enabled to achieve resectability rate of 100% and 5-year survival rate as high as 90.3% in patients of early esophageal cancer. With reasonable early diagnosis and refined operative technique with less postoperative complications can lead to achieve high resectability, low operative mortality and favorable survival rate.
