MICRONEUROVASCULAR RECONSTRUCTION IN PLASTIC AND RECONSTRUCTIVE SURGERY

Department of Plastic and Reconstructive Surgery, Keio University School of Medicine

Toyomi Fujino

In plastic and reconstructive surgery, free cutaneous flap and free myocutaneous flap have mainly used recently for microvascular reconstruction.
Historically, the first experimental success of free cutaneoud flap was done by Krizek in 1965, and in Japan by Fujino in 1966. Experimental success of free mammary gland composite tissue transfer was first performed by Fujino in 1966. Clinically our success was in 1974, mainly due to improvements of microvascular suture instruments, materials and methods. Our eldest age for success was 70 year old.
Using the myocutaneous flap, we have been treated the congenital absence of the breast as well as the secondary loss of the breast after radical mastectomy. Contrary to those static reconstruction, we are now in practice in functional vs dynamic reconstruction of the missing structures by free mvocutaneous neurovascular flap.
